Authorization for Emergency Medical Care

1. Emergency Medical Policy

In case of emergency illness or accident the child is given first aid and the parents are notified. If the parents cannot be located, the child will be taken to Mildmay Hospital, Lweza. Elami International Christian School (EICS) does not assume responsibility for the payment of hospital, doctor or transportation fees.

If I cannot be reached to plan for emergency medical care at the time of an accident or illness, I hereby authorize Elami International Christian School (EICS) to take my child to Mildmay Hospital, Lweza
or

If no hospital is specified, my child will be taken to Mildmay Hospital, Lweza.

2. Insurance Waiver

Elami International Christian School (EICS) has liability insurance, which will pay for medical costs (up to predetermined limit of UGX 100,000) if your child is injured while at school, at any school related activity or while being transported in school vehicles.
I understand that Elami International Christian School (EICS) will not be held responsible for any injury that occurs involving my child(ren). The school’s liability insurance will pay for medical costs up to the designated limits, and I will not expect Elami International Christian School (EICS) to pay beyond that.

Any incomplete or false information written on student health records will result in non-coverage of health insurance.
